Eligible family members encompass spouses and any dependent children under 19 years old (or under 21 if the dependent child is a full-time student). Secure your membership by paying the full plan fee upon registration; please note that fees are non-refundable.
Your membership spans a year from the date of your registration/payment, ensuring continuous benefits for you and your family. Discounts apply exclusively throughout the 12-month term.
About Desert Sage Dental
Since 2008, Desert Sage Dental has grown to meet patient needs, adding CEREC® one-visit crowns and implants in 2009. By 2010, we relocated to support growth. Dr. Zentz became a full-time partner in 2014, and in 2024 we welcomed Dr. Ong, adding fresh expertise.
